Community Mental Health Team

Slough Pathways

Age group served: 18 - 75 years old

We are a multidisciplinary team of professionals including community psychiatric nurses, social workers, support workers, psychologist, psychiatrists and occupational therapists. We provide a range of services for people requiring secondary mental health services, including mental health and social care assessments; coordination of mental health care; effective treatments for mental health conditions and social care support for service users and their carers. We also provide support with housing and access to benefits information for people receiving secondary mental health services, and can assist service users to access practical support at home.

In addition to the above CMHT services, Slough Pathways also includes specialist workers for early intervention in psychosis, assertive outreach provided by the Slough Pathways Outreach Team (SPOT), dual diagnosis worker, and psychological therapies.

Early intervention in psychosis: specialist workers provide a service for young adults who are experiencing their first episode of a psychotic illness. We educate and provide early treatment and prevention of relapse to service users and their carers.

SPOT (Slough Pathways Outreach Team): this service provides intensive support to people with severe and enduring mental illness combined with other complex needs such as a history of hospital admissions and multiple social problems.

Psychological therapies: a range of therapies are available including cognitive behaviour therapy; systemic family therapy; and dialectical behaviour therapy. Therapy can be an alternative or accompaniment to medication. Treatments are provided for groups or individuals by a team of clinical psychologists, clinical nurse specialists and trainees.

Common conditions treated: services are for people with severe and / or enduring mental health conditions including: bipolar disorder; clinical depression; obsessive compulsive disorder (OCD); severe anxiety disorders; personality disorders; schizophrenia; serious self harm; dual diagnosis. In addition we can refer to specialist services for conditions such as eating disorders and post traumatic stress disorder.
